Registration forms, waivers and emergency info (Campus)
Programs collect registration questions and signed waivers before enrollment; families keep emergency contacts and allergies where staff can see them.
Programs with Campus have a Forms tab.
Make a form (Administrators)
- On Forms, tap New form.
- Name it and choose Questions to answer (like a registration form) or A waiver or agreement to sign (paste its text).
- Choose whether it's filled in once per family or for each student, and whether it's for every term or one term.
- Add questions: short answer, paragraph, yes or no, or pick one (one choice per line). Tick Required for the ones that must be answered.
- Leave Required before enrolling ticked if families must finish it before they can enroll a student. Tap Create form.
Turn off hides a form from families and stops requiring it, keeping its answers. Edit changes it; families who already filled it in keep their answers and the version they signed. Delete this form removes it and its answers.
See who's done (Administrators and office staff)
Tap N answered next to a form for everyone's answers and signatures (with the date and version signed) and a Still to go list. Print or save as PDF keeps a copy.
Fill them in (parents)
Open the program's Forms tab. Your forms lists each form (for each student where needed) as Done, Needed or Optional. Tap Fill in or Read and sign; a waiver is signed by ticking that you agree and typing your full name. You can come back and change your answers. Until the required ones are done, Classes reminds you, and enrolling a student says which forms are left.
Emergency info (parents)
On My family, under Emergency info, add up to four emergency contacts and, for each student, allergies and medical notes. Only your family and the staff of your Campus programs see them: allergies show next to your student at check-in and on their class lists, and staff can open the rest when they need it.
Up to date as of October 31, 2026.
